Categories missing after installation

Howdy all!

I've been working to install osclass in a test environment and have it up and running now. Unfortunately when I go into the "Categories" screen it is empty. Pressing the "+" button in the upper right hand corner causes the page to load, but I can't add a category. Looking in the database I can see that the three tables to hold the category data are there and populated; I'm not sure if the data is correct though. There are no obvious errors in the app logs or in the browser.

Any thoughts on what could be wrong? I can post my docker files to recreate my environment. I should note that I am running the latest version, 3.7.4.
